We have outlined the basic rehabilitation process below to help you better understand what to expect during your stay. The length of time will depend on individual circumstances but the general process will be the same.
Your stay begins with evaluations completed by nursing, physical therapy, occupational therapy and speech therapy staff, and a physician.
Based on your admission evaluation, the team will work with you to develop an individualized plan of care. You will have three hours of therapy at a minimum of five days per week. You will have rest breaks, and the schedule will be modified to meet your individual needs. Each week, a team discusses progress toward goals, and barriers to discharge are evaluated. The case manager will review any treatment plan changes and will meet with you following this meeting. We may also schedule a meeting for you and your family and/or caregiver to discuss goals, barriers and/or questions about your treatment.
In addition to receiving ongoing training with nursing and your therapy, each week you will receive team conference updates from your case manager and review your updated goals and discharge dates. If needed, we will discuss equipment needs for discharge and arrange for its order. We will continue to review and discuss any services needed after your discharge and note your preferences as to where to obtain those services. We will assist you in the completion of necessary paperwork and continue to meet with family/caregivers as needed.
